Sam D. Toney, M.D.

A board-certified psychiatrist, Dr. Sam Toney founded Health Integrated in 1996 in response to the growing need for sophisticated care management systems that address both the medical and behavioral health issues impacting patient care and outcomes. 

He has developed a number of patient care programs including partial hospitalization programs and substance abuse initiatives for a variety of clinics. 

Dr. Toney also developed care management programs for multi-specialty IPA and Primary Care Networks throughout the country. He designed one of the nation’s first comprehensive depression disease management programs and then went on to develop Health Integrated’s comprehensive medical/behavioral condition management program (MCCM). 

He is the architect of Dynamic Somato-Social Theory which serves as the behavior modification protocol supporting all of Health Integrated’s products and services. 

Other clinical initiatives include the development of extensive behavioral health medical necessity criteria, pharmaceutical criteria and psychiatric case management programs.  

Dr. Toney is licensed to practice medicine in 35 states and is board certified in Psychiatry as well as in Utilization Review and Quality Assurance.
